- Form-6 with Declaration Form (Annexure-IV): For registration of new voters.
- Form-7: For deletion of names from the electoral roll.

All submissions must be made by March 6, 2026, and can be directed to the office of the Electoral Registration Officer or the Booth Level Officer (BLO) of the relevant part number.
