The tragic collapse of a 10-lakh litre capacity water tank at Avaada Group's solar panel manufacturing plant in Nagpur, which resulted in the deaths of six workers and left nine injured on December 19 last year, has been attributed to gross negligence, according to a recent investigation. FIR Registered Against Project Heads
Earlier this week, an FIR was registered under bailable provisions for causing death due to negligence. The accused include Prashant Komal Arvind Kumar, the project head at Avaada Group, and Rajesh Yadav, the project head at Goyal Steel Tech, the associated contractor. Initially, the Butibori police had filed an accidental death case, but this was escalated following a comprehensive report from the Directorate of Industrial Safety and Health (DISH), Nagpur, submitted on January 20.

The investigation uncovered that the tragedy was a direct result of negligence and carelessness during the tank's assembly and testing phases. Critical issues identified include:
- Loosely fitted nuts and bolts, compromising the structural integrity of the tank.
- Inadequate safety protocols during construction and pressure testing.
- Failure to ensure structural integrity under operational pressure, leading to the catastrophic collapse.
