Untrained Operator and Overdue Pressure Tests Identified
Officials discovered that the operator responsible for running the thermic fluid heater was not a trained professional, a violation of basic safety protocols. Additionally, the unit had failed to conduct a mandatory pressure test on the heater's coil, which was overdue by three months. These oversights contributed directly to the hazardous conditions that led to the fire.
"We found the pump transmitting oil to the heater circuit developed a leak, and smoke billowed when the oil caught fire after contact with air," an official noted. "The deceased worker attempted to close the valve of the pump, which unfortunately aggravated the situation. Had a trained operator been present, this tragedy could likely have been averted."
