Telangana Ramps Up HIV Control with Mission AIDS Suraksha Initiative

In a significant public health push, Telangana has launched Mission AIDS Suraksha (MAS), a comprehensive strategy aligned with National AIDS Control Organisation (NACO) guidelines to meet global HIV targets. The initiative represents a focused effort to tackle the state's HIV epidemic through targeted interventions in high-priority districts.

19 Districts Identified for Intensive HIV Control Measures

The health department has designated 19 districts as priority areas requiring concentrated efforts. These include Hyderabad, Rangareddy, Siddipet, Sangareddy, Peddapalli, Warangal, Medak, Adilabad, and Khammam. Officials emphasize that these districts collectively account for 96% of people living with HIV and encompass all high-risk populations within Telangana.

According to health department data, these priority districts show an overall HIV incidence rate of 0.12% and a prevalence rate of 0.47%, indicating what experts describe as a moderate but concentrated epidemic requiring specialized attention.

Aligning with UNAIDS 95-95-95 Strategy

Mission AIDS Suraksha specifically targets the UNAIDS 95-95-95 goals, which aim for:

  1. 95% of people with HIV knowing their status
  2. 95% of those diagnosed receiving antiretroviral treatment
  3. 95% of those on treatment achieving viral suppression

Currently, Telangana's performance stands at 85-93-98, indicating the state is particularly lagging in identifying all HIV-positive individuals. A senior health department official explained the challenges: "What we are lagging behind in is identifying all HIV-positive individuals, possibly due to low OPD-based HIV testing and underperforming facilities, limited community-based screening in uncovered areas, and inadequate coverage of tribal, prison and migrant populations."

Addressing Systemic Challenges in HIV Control

The initiative will specifically tackle several persistent issues that have hampered previous HIV control efforts:

  • Weak TB-HIV coordination and limited follow-up of partners of those testing positive
  • Delays between HIV diagnosis and therapy registration, often caused by stigma, fear, and insufficient counseling
  • Inadequate screening coverage in tribal areas, prisons, and migrant populations
  • Underperforming testing facilities and limited community-based screening programs

S Sangeetha Satyanarayana, Commissioner of the Health and Family Welfare Department, outlined the strategic approach: "The strategy focuses on identifying every undiagnosed case and ensuring treatment to curb transmission. To achieve this, focused screening camps in tribal areas, prisons and migrant clusters, along with mapping of high-risk locations such as tourism areas and massage parlours, will be conducted."

Current HIV Statistics and Progress in Telangana

According to the Telangana State AIDS Control Society (TGSACS), the state currently has over 1.56 lakh persons living with HIV (PLHIV), with approximately 10,000 new cases registered annually. Despite these numbers, officials report significant progress over the past two years, with Telangana moving from 33rd to 10th position nationally in NACO's HIV control indicators.

The state is meeting its annual screening target of around 17 lakh tests set by NACO, with increasing emphasis on community-based screening. Additionally, the HIV prevalence rate has shown a decline from approximately 0.44% in 2023 to 0.41% in 2025.

Ambitious Timeline for HIV Elimination

The health department has set an ambitious target of achieving zero new HIV infections by December 2026, with efforts continuing through December 2027. Medical experts emphasize that achieving viral suppression through consistent treatment is crucial to stopping HIV transmission, making this a key focus of Mission AIDS Suraksha.

The comprehensive approach combines enhanced testing, improved treatment access, community engagement, and targeted interventions in high-risk areas to create a multi-pronged strategy against HIV transmission in Telangana.
